A GIFT INTENTION FORM IS AVAILABLE HERE
The form is divided into four sections: Contact Information, Gift Intention, Form of Giving, and
Gift Designation.
Contact Information
It is very important that we keep you informed as we progress in our building process with
timely updates and giving statements. If you have questions about your current contact
information or giving, please contact Navajo Ministries at 505.325.0255.
Gift Intention Planning
Funds for the Navajo Heritage Center Celebration of The People Campaign are needed
beginning June 2008 and continuing over the next three years, through May 2011. For your
convenience, we have provided several different payment options to help you plan and
calculate your gift intention over the next three years.
Form of Giving
Navajo Ministries or Navajo Heritage Center can accommodate several different forms of
payment.
 Cash Check

Or, should you prefer, we will contact you to arrange the details for:
Automatic bank withdrawal
Stock, real estate, insurance, or estate assets
In-Kind donation of services or products
We are also happy to provide special return envelopes for your gifts. Please mark the box on
the Gift Intention Form to request the envelopes. It is also important to clearly indicate on
your checks the Celebration of The People Campaign designation in order to ensure your
gifts are used as you intend.
Gift Designation
Your gift(s) can be designated to sponsored opportunities as well as to either Navajo
Ministries, Inc., an established 501(C)3 organization, or Navajo Heritage Center, 501(C)3
applied for. At this time, your gifts to Navajo Ministries, Inc. are fully tax deductible. The Gift
Intention Form provides the area to designate your gift to either organization and to a
sponsored opportunity.
